We don’t want you tobe a statistic.As part of our commitment to your health and long-term recovery, we’d like to offer you care coordinationservices.Research shows that patients who make and keep their follow up appointment are more likely to stay out of thehospital and feel better.Our care coordinator will talk to you before your discharge to set up a follow up appointment with your doctor.At your follow up appointment, your primary care will review your medications and discuss your recovery andtreatment plan so you can regain and maintain your health.“Following the discharge plan was key, it was instrumental toward the recovery. And the plan wasstaying in communication with them around any questions.”“She’s gonna make an appointment with you and be prepared to sit down with her and go through whatall’s been done.
